We are looking for a dedicated Home Care LPN to join ChristianaCare's forward-thinking HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, ensuring that care plans are patient-centered and aligned with available resources.

Responsibilities

• Administer skilled nursing care and treatments to geriatric patients in the community setting.
• Develop and implement patient-centered care plans in collaboration with the healthcare team.
• Participate in discharge planning to ensure continuity of care.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
