Job Description
We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. This is a great opportunity for a skilled Licensed Practical Nurse to provide quality care to geriatric patients in the community. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
Job Types: Full-time, Part-time, Per diem
Pay: $22.00 - $28.00 per hour
Expected Hours: 20 – 40 per week
