Job Description
We are looking for an experienced Home Care LPN to join the ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
