Are you an experienced Home Health Licensed Practical Nurse (LPN) looking to join a team that prioritizes professional development and exceptional patient care? ChristianaCare HomeHealth is seeking dedicated LPNs to deliver skilled nursing care to geriatric patients in the community.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in line with available resources.
- Collaborate to develop effective discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
