Job Description

Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement goal-oriented care plans that are patient-centered and resource-consistent.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
