Job Description
We are looking for an experienced Home Care LPN to join our dynamic team at ChristianaCare. As a valuable member of our organization, you will be providing skilled nursing care to geriatric patients in their homes. Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients in the community.
- Implement patient-centered care plans that align with available resources and patient goals.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

Schedule
