Job Description
We are looking for a dedicated Home Care LPN to join ChristianaCare's HomeHealth team. As an experienced licensed practical nurse, you will provide skilled nursing care and treatment to geriatric patients in the community. Responsibilities
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
- Work flexible hours supporting New Castle County, DE.
Requirements
- HSD or GED required.
- Must possess a current DE LPN license.
- Experience in geriatrics, home health, pediatrics, or wound care is a plus.
