Job Description
We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to the discharge planning process. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
