Job Description
We are looking for an experienced Home Care LPN to join the ChristianaCare HomeHealth team. As a Licensed Practical Nurse, you will provide skilled nursing care and treatment to geriatric patients, implement care plans, and contribute to discharge planning. This position offers flexible work options, including full-time, part-time, and per diem roles, with competitive pay ranging from $22.00 to $28.00 per hour.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
