Job Description
We are looking for experienced Home Care LPNs to join our dedicated team at ChristianaCare. As a Home Care LPN, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in discharge planning. This role offers a flexible schedule, competitive pay ranging from $22.00 to $28.00 per hour, with expected hours between 20-40 per week.
Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented and patient-centered.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
