Job Description
We are seeking experienced Home Care LPNs to join our team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, implement goal-oriented care plans, and contribute to patient discharge planning. Full-time, part-time, and per diem positions available with competitive pay ranging from $22 to $28 per hour.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Develop and implement patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
