Job Description
We are looking for an experienced Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented and patient-centered.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
