Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a Home Care LPN, you will be responsible for providing skilled nursing care to geriatric patients in the community. This role involves implementing care plans, participating in discharge planning, and ensuring patient-centered care with available resources.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
