Job Description

We are looking for a dedicated Licensed Practical Nurse (LPN) to join Genesis Health's team. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure high-quality patient care and proper nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument process, ensuring compliance with guidelines and regulations.
• Develop individualized resident Care Plans and identify resident needs and outcomes.
• Train new team members on clinical content and electronic MDS completion.

Requirements

• LPN in good standing and currently licensed.
• Prior experience in healthcare settings is required.
• Proficient in general nursing practices and MDS completion.
• Computer skills including Windows applications.
• AccuMed experience is a plus.
• BLS/CPR certification required.
