Job Description

Genesis Health is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing necessary items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure high-quality patient care and proper reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process, ensuring accurate completion, encoding, and electronic transmission of the MDS.
• Contribute to the development of individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare setting (hospital, long-term care, etc.).
• Familiarity with general, rehabilitative, and restorative nursing practices.
• Proficiency in computer skills, including Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• Current BLS/CPR certification.
