Job Description
Genesis Healthcare is seeking a Licensed Practical Nurse (LPN) to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process through the completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure high-quality patient care and nursing center reimbursement.
Position Highlights
- Direct the interdisciplinary RAI process and ensure accurate completion, encoding, and transmission of the MDS.
- Develop individualized resident Care Plans in compliance with Federal and State regulations.
- Provide training on clinical content and electronic completion of MDS items to new team members.
Requirements
- LPN in good standing with current state license.
- Prior experience in hospital, long-term care, or related healthcare settings.
- Knowledge of nursing practices, medical procedures, and long-term care regulations.
- Computer proficiency, including Windows applications.
- AccuMed experience preferred.
- Previous experience as an MDS Coordinator is a plus.
- BLS/CPR certification required.
