We are looking for a dedicated Licensed Practical Nurse (LPN) to join our team at Genesis Healthcare. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process through the completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs). Your responsibilities will include directing the interdisciplinary RAI process, developing individualized resident Care Plans, and training team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Prior experience in healthcare facilities.
