Job Description
We are currently seeking a Licensed Practical Nurse (LPN) to join our team at Genesis HealthCare. As an LPN MDS Coordinator, you will be responsible for participating in the Resident Assessment Process, completing the Minimum Data Set (MDS), and ensuring high-quality patient care and appropriate reimbursement.
Position Highlights
- Direct and oversee the Resident Assessment Instrument (RAI) process.
- Develop individualized Care Plans for residents in compliance with Federal and State regulations.
- Provide training on clinical content and electronic completion of MDS items.

Requirements
- LPN license in good standing.
- Prior experience in healthcare facilities.
- Knowledge of nursing practices and regulations.
- Computer skills with proficiency in Windows applications.
- AccuMed experience is a plus.
