Job Description
We are seeking experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community while implementing patient-centered care plans. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.

Full-time, Part-time, Per diem
Pay
$22.00 - $28.00 per hour
Expected Hours
20 – 40 per week
