Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. As an LPN in HomeHealth, you will provide skilled nursing care, implement care plans, and support geriatric patients in the community. Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
Job Details
- Job Types: Per diem, Part-time, Full-time
- Pay: $22.00 - $28.00 per hour
- Hours: 20 – 40 per week
- Medical Specialties: Geriatrics, Home Health, Pediatrics, Wound Care
- Shifts: Day shift, Night shift
- Supplemental pay types: Differential pay
- Supplemental schedule: Holidays, Overtime
- Weekly schedule: 5x8, Monday to Friday, Rotating weekends
