Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) in home care to join our team at ChristianaCare. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented care plans that are patient-centered and resource-appropriate.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
