Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join ChristianaCare HomeHealth and make a difference in the community. Our team provides skilled nursing care to geriatric patients with a focus on goal-oriented and patient-centered treatment plans.
Responsibilities
- Deliver skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented care plans consistent with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current Delaware LPN license or compact state license.
