Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare Home Health team. If you are passionate about providing skilled nursing care to patients in the community and are dedicated to patient-centered care, then we want you to be part of our organization.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
