Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) in Home Care to join our team at ChristianaCare. If you are dedicated to providing quality care to geriatric patients in the community, we have opportunities for Full-Time, Part-Time, and Per Diem LPNs. In this role, you will be responsible for administering skilled nursing care, implementing care plans, and participating in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED is required.
- Must possess a current DE LPN license or compact state license.
