We are looking for an experienced Licensed Practical Nurse (LPN) to join our Home Care team at ChristianaCare. Responsibilities
- Deliver skilled nursing care and prescribed treatments to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.

Work Details
- Job Types: Per diem, Part-time, Full-time.
- Pay: $22.00 - $28.00 per hour.
- Expected hours: 20 – 40 per week.
- Specializing in Geriatrics, Home Health, Pediatrics, and Wound Care.
- Shifts: Day shift, Night shift.
- Weekly schedule: 5x8, Monday to Friday, Rotating weekends.
- Work Location: In person.
