LPN Home Care - $22-$28 / hr

Job Description

We are looking for experienced Licensed Practical Nurses (LPNs) to join our Home Care team at ChristianaCare. In this role, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and contributing to discharge planning.

Responsibilities

• Deliver high-quality nursing care to patients based on individualized care plans.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Participate in developing and implementing patient discharge plans.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
