Job Description
LPN Home Care - $22-$28 / hr
We are seeking experienced Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN with us, you will provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning, all while working in a supportive and well-equipped environment.
Duties and Responsibilities
- Give skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement care plans that are goal-oriented and patient-centered.
- Participate in developing discharge plans for patients.
Requirements
- HSD or GED required.
- Must possess a current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
