Job Description
ChristianaCare HomeHealth is seeking dedicated and experienced LPNs to join our team for Full-time, Part-time, and Per Diem positions. As an LPN in our HomeHealth program, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to patient discharge planning. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
