Job Description
We are looking for experienced and dedicated Licensed Practical Nurses (LPNs) to join our dynamic team at ChristianaCare HomeHealth. As an LPN in our organization, you will have the opportunity to provide skilled nursing care and treatment to geriatric patients in the community, while being supported by our state-of-the-art Electronic Health Record system and comprehensive training programs.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
