Job Description
ChristianaCare HomeHealth is seeking dedicated Licensed Practical Nurses (LPNs) to join our team in providing skilled nursing care to geriatric patients in the community. As a member of our team, you will have the opportunity to implement care plans, contribute to discharge planning, and support patient-centered care.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
