We are seeking Full Time, Part Time, and Per Diem LPNs to support patients in New Castle County, DE. The role involves providing skilled nursing care, implementing care plans, and participating in discharge planning.
Responsibilities:
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-focused care plans with available resources.
- Participate in developing discharge plans.
Requirements:
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
Job Types: Per diem, Part-time, Full-time
Pay: $22.00 - $28.00 per hour
Expected hours: 20 – 40 per week
