Genesis HealthCare, a leading provider of healthcare services, is seeking an LPN MDS Coordinator to join their team. The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper reimbursement.

Position Highlights

• Direct and oversee the RAI process, ensuring compliance with guidelines and regulations.
• Develop individualized resident Care Plans based on resident needs and expected outcomes.
• Train interdisciplinary team members on clinical content and electronic MDS completion.

Requirements

• LPN license in good standing.
• Prior experience in healthcare facilities.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Computer proficiency, preferably with AccuMed.
• Experience in MDS completion and Care Plans.
• Proficiency in RAI/PPS processes.
• Current BLS/CPR certification.
