Position Highlights

• Lead the Resident Assessment Process through MDS completion for appropriate patient care and nursing center reimbursement.
• Direct interdisciplinary RAI process adherence and train team members in clinical content and electronic MDS completion.
• Develop individualized resident Care Plans based on Federal and State regulations.

Requirements

• Active LPN license with relevant experience in healthcare facilities.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer skills, including windows applications, and preferably AccuMed experience.
• Prior MDS Coordinator experience and demonstrated RAI/PPS process proficiency.
