The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process and ensure MDS completion adheres to guidelines.
• Develop individualized resident Care Plans according to Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to team members.

Requirements

• Current LPN license with prior experience in healthcare facilities.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Proficiency in AccuMed and demonstrated experience in MDS completion.
• BLS/CPR certification required.
