Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is currently seeking a dedicated and qualified Licensed Practical Nurse to join our team as a Minimal Data Set Coordinator. As the LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Oversee the interdisciplinary Resident Assessment Instrument (RAI) process, adhering to completion guidelines and electronic transmission of the MDS.
• Contribute to the development of personalized resident Care Plans, addressing resident needs and expected outcomes in compliance with Federal and State regulations.
• Train new team members on clinical content and electronic completion of relevant MDS items.

Requirements

• Active LPN license in good standing.
• Prior experience in a hospital, long-term care, or related healthcare facility.
• Familiarity with general, rehabilitative, and restorative nursing practices, as well as relevant laws and regulations.
• Proficiency in Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience advantageous.
• Current BLS/CPR certification required.
