Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a skilled Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team at Genesis Health. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure quality patient care and proper reimbursement. Position Highlights

• Lead the RAI process following guidelines for completion, encoding, and transmission of the MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train interdisciplinary team members on clinical content and electronic completion of MDS items.

Requirements

• Current LPN licensure in good standing.
• Prior experience in healthcare facilities.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer applications, including Windows.
• AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
• BLS/CPR certification required.
