Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is seeking a dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential sections of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Direct and oversee the Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans following Federal and State regulations.
• Provide training on MDS items completion to the interdisciplinary team members.

Requirements

• Active LPN license.
• Prior experience in healthcare settings.
• Proficiency in general nursing practices and medical procedures.
• Computer skills, including Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience preferred.
• BLS/CPR certification required.
