Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis HealthCare is seeking a dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team. As a crucial member of our healthcare services, you will be responsible for participating in the Resident Assessment Process by completing essential sections of the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and appropriate nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in compliance with guidelines for MDS completion, computer encoding, and electronic State transmission.
• Aid in developing personalized resident Care Plans, identifying resident needs and expected outcomes as per Federal and State regulations.
• Train new team members on the clinical specifics and electronic completion of relevant MDS items.

Requirements

• LPN in good standing, currently licensed by the state.
• Prior experience in a healthcare facility is necessary.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficient in using computer applications, particularly Windows.
• AccuMed experience is a plus.
• Previous MDS Coordinator experience is preferred.
• Strong expertise in RAI/PPS processes.
• Current BLS/CPR certificate is required.
