Genesis Health is seeking a skilled and dedicated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team. As an LPN MDS Coordinator, you will play a critical role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the nursing center's Resident Assessment Instrument (RAI) process in compliance with guidelines for MDS completion, encoding, and electronic State transmission.
• Contribute to the development of personalized resident Care Plans, identifying resident needs and expected outcomes in adherence to Federal and State regulations.
• Train new interdisciplinary team members on clinical content and electronic MDS completion.

Qualifications

• Active LPN license.
• Prior experience in healthcare facilities.
• Knowledge of nursing practices, medical procedures, and relevant regulations.
• Proficiency in computer applications, preferably AccuMed.
• Previous MDS Coordinator experience preferred.
• BLS/CPR certification required.
