Job Description: Minimal Data Set Coordinator - Licensed Practical Nurse

Genesis Health is seeking a detail-oriented Licensed Practical Nurse (LPN) to join our team as a Minimal Data Set (MDS) Coordinator. In this role, you will play a vital part in the Resident Assessment Process by completing MDS elements and Care Area Assessments (CAAs) to ensure quality patient care and accurate nursing center reimbursement.

Position Highlights:

• Lead the interdisciplinary RAI process, ensuring accurate MDS completion and electronic transmission.
• Create personalized resident Care Plans in compliance with Federal and State regulations.
• Train team members on clinical content and electronic MDS completion.

Requirements:

• Active LPN license in good standing.
• Prior experience in healthcare settings.
• Proficiency in general nursing practices and medical procedures.
• Computer literacy, including Windows applications; AccuMed experience preferred.
• Previous MDS Coordinator experience and BLS/CPR certification required.
