Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

We are looking for a motivated and skilled Minimal Data Set Coordinator - Licensed Practical Nurse to join Genesis Health, a leading provider of healthcare services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the RAI process and ensure accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• Current LPN license in good standing.
• Prior experience in hospital or long-term healthcare settings.
• Proficiency in general nursing practices, medical procedures, and long-term care regulations.
• Computer skills, including knowledge of Windows applications.
• AccuMed experience preferred.
• Previous MDS Coordinator experience is a plus.
• Current BLS/CPR certification.
