Minimal Data Set Coordinator - Licensed Practical Nurse

Job Description

Genesis Health is looking for a dedicated Licensed Practical Nurse to join our team as a Minimal Data Set Coordinator. As an essential part of our healthcare services provider, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and reimbursement for our nursing center.

Position Highlights

• Direct and oversee the Resident Assessment Instrument (RAI) process in compliance with guidelines.
• Contribute to the development of individualized resident Care Plans.
• Provide training to new team members on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Prior experience in hospital, long-term care, or healthcare setting.
• Familiarity with nursing practices, medical procedures, and regulatory guidelines.
• Proficiency in computer applications, including Windows.
• AccuMed experience is a plus.
• Previous experience as an MDS Coordinator preferred.
• Strong knowledge of RAI/PPS processes.
• Current BLS/CPR certification.
