Nurse-LPN MDS Coordinator

Job Description

We are looking for a dedicated Nurse-LPN MDS Coordinator to join Genesis Healthcare, a leading provider of healthcare services. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines and electronic transmission of MDS.
• Contribute to the development of personalized resident Care Plans to meet Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new interdisciplinary team members.

Requirements

• Active LPN license.
• Prior experience in healthcare facilities.
• Knowledge of nursing practices and regulations in long-term care.
• Proficiency in windows applications and familiarity with AccuMed is advantageous.
• Previous experience with MDS completion and Care Plans is preferred.
• Current BLS/CPR certification.
