Responsibilities

• Conduct home visit assessments of referred patients.
• Assess medical conditions and functional limitations.
• Develop and implement personalized treatment plans.
• Utilize EMR documentation for accurate record-keeping.

Requirements

• Graduate of an accredited Physical Therapy program.
• Active Physical Therapy license for New York.
• Proficient in EMR documentation (Hello Note experience a plus).
• CPR Certification.
