Responsibilities

• Conduct home visit Physical Therapy assessments for patients.
• Provide direct patient care to evaluate medical conditions and potential for rehabilitation.
• Proficiently document patient information using EMR systems.
• Maintain CPR certification.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Experience in conducting assessments and providing direct patient care.
• Proficiency in EMR documentation preferred.
