• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Conduct home visit assessments, providing direct patient care and evaluating medical conditions for rehabilitation potential.
• Proficient in EMR documentation.
• Hold a valid CPR Certification.
