Responsibilities

• Conduct home visit Physical Therapy assessments, providing direct patient care and assessing medical conditions.
• Utilize EMR documentation to track patient progress.
• Collaborate with patients to improve functional capabilities and promote rehabilitation.

Requirements

• Graduate of an accredited Physical Therapy program.
• Unencumbered Physical Therapy license for New York.
• CPR Certification.
