Paradigm Rehab is currently seeking a compassionate and dedicated Physical Therapy Specialist to join our team. In this role, you will have the opportunity to help patients improve their quality of life by providing direct care and rehabilitation services.

Physical Therapist Responsibilities:

• Conduct home visit assessments to evaluate patients' medical conditions and functional capabilities.
• Provide direct patient care to assist in the rehabilitation process.
• Document patient records using EMR systems.
• Maintain a current CPR certification.

Requirements:

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license in New York.
• Proficient in EMR documentation.
• Current CPR Certification.
