Physical Therapy Specialist Responsibilities:

• Conduct home visit assessments to evaluate medical conditions and functional capabilities.
• Provide direct patient care to assess limitations and rehabilitation potential.
• Proficient in EMR documentation (experience with Hello Note is a plus).
• Maintain an unencumbered Physical Therapy license for New York.

Physical Therapy Specialist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold a CPR Certification.
• Competitive pay rates of $70 - $90 per visit.
