Job Description

We are looking for a dedicated and experienced Registered Nurse (RN) to join our Home Care team at ChristianaCare. As a Field Nurse, you will provide skilled nursing care to geriatric patients in the community, ensuring quality treatment and patient-centered care plans.

Responsibilities

• Deliver skilled nursing care and prescribed treatments to geriatric patients in the community setting.
• Develop and implement patient-centered care plans consistent with available resources.
• Ensure IV proficiency and perform IV functions as required.
• Develop discharge plans for patients.

Requirements

• BSN required; ADN candidates must obtain BSN within three years of start date.
• Current DE RN license or compact state license.
• 2 years of recent HomeHealth RN experience.
• Previous experience in Home Healthcare and Med-Surgical preferred.
• Willingness to train and perform IV functions.
